WAIVERS AND DISCLOSURES

1. I am legally permitted to work in Canada, and I am not taking any Social Assistance Benefits or Employment Insurance Benefits
2. The Employer/Client will record your working hours on a time sheet at the workplace. It is YOUR responsibility to keep track of your hours. to sign in and sign out. (Failure to do so may result in loss of pay from missing hours) If you are not satisfied with the assignments given by the employer or GTA Property Care, please immediately notify the employer and call our office to inform us about the situation. If you walk out of the job without informing us and the Company/Client, it will result in a reduction or loss of hours. In case you are not on time, you must inform the Employer As well GTA Health Care in Advance.
3. I understand that I may be subject to a background check, you are hereby authorized to make any investigation about my criminal record and employment record. I authorize GTA Property Care to do so.
4. You must follow the company health and safety procedures and follow all the guidelines provided by the employer or agency

EMPLOYMENT TERMS AND CONDITIONS

1. In case of emergency sickness leave, please inform supervisor and agency as soon as possible.
2. Cell phone use in the In workplace and communication amongst each other during the working hours is strictly prohibited
3. You must claim missing hours no later than 2 weeks and pick up your pay within 90 days; otherwise GTA Health Care will not be responsible
4. In case of disrespectful or short-tempered behavior at work, you will be terminated immediately. If someone else misbehaves with you, please inform your boss immediately, as well as GTA Property Care
5. You must follow the guidelines regarding health and safety which come from Government body or Employer.
6. I allow GTA Property Care to have my picture to be stored in their internal records for recognition and ID purposes.
